Please review the waiver in full before completing this form. By submitting, you are providing your electronic signature and acknowledging the waiver terms.

  • This waiver is required for court reservation access, but does NOT create any membership, ownership, priority, or entitlement.

  • Access may be revoked at any time at the discretion of the property’s authorized administrator, to the extent permitted by law.

  • If you are submitting on behalf of a minor, a parent/legal guardian must sign and provide the minor’s name.
